- Illustrated limitation page appearing on the last page before the back loose endpaper bears the statement "The edition is limited to 300 signed and numbered copies. This is copy number" below which the number of the copy is printed (not handwritten) in a much larger font. This is followed by the signatures of Richard Lupoff (the introducer), John Pelan (the editor and author of the afterword), and Jacob McMurray (the cover artist/designer).
- "February 2017" stated on the copyright page.
- The price is not stated in the book. Copies from the publisher came shrink-wrapped with a sticker on the back printing the barcoded ISBN and the price of $100.00. There was also a sticker printing the number of the copy.
- "The Collected Short Fiction" (over) "Volume 4" (over) "Introduction by Richard Lupoff" (over) "Edited, with an afterword, by John Pelan" on the title page.
