Notes: Limited edition of 1000 numbered copies. The introduction is listed in the table of contents as beginning on page v. However, no pages bear Roman numbers and it is not the 5th page of the book. Additionally, the second page of the introduction would be on the Arabic numbered page 1 were that page numbered. There is an additional plate between pages 68 and 69 in the first verified copy that is blank. It is unclear whether this is intentional or a printing error.
